Understanding Elbow Tendinopathy
Tennis elbow and golfer's elbow are caused by overloading the tendons that attach to the outside (lateral) or inside (medial) of the elbow. This overload leads to changes within the tendon that cause pain, weakness, and difficulty gripping. These conditions are most common in people aged 35-55 and are frequently caused by repetitive work tasks, computer use, or DIY activities — not just racket sports. Rest alone rarely resolves the problem; the tendon needs specific, progressive loading to recover.
Progressive Loading Is Key
The gold standard treatment for elbow tendinopathy is a progressive tendon loading programme. This starts with isometric exercises (static holds) to manage pain, then gradually progresses through eccentric and heavy slow resistance exercises to rebuild tendon strength and resilience. I also address contributing factors such as grip strength, wrist and forearm flexibility, and any workplace or sporting technique issues. Most people see significant improvement within 8-12 weeks of a consistent programme.
Common Symptoms
- Pain on the outside or inside of the elbow
- Pain when gripping, twisting, or lifting
- Weakness in grip strength
- Morning stiffness around the elbow
- Pain that worsens with repetitive arm movements
- Tenderness when pressing on the elbow
