Muscle Strains & Tears
Muscle strains and tears are common in both sport and everyday life. The right rehabilitation approach is crucial — it determines how quickly you recover and whether the injury comes back.

How Muscle Injuries Occur
Muscle strains occur when muscle fibres are stretched beyond their capacity, causing microscopic or macroscopic tears. They are graded from 1 (mild) to 3 (complete rupture). The hamstrings, quadriceps, calf, and groin muscles are most commonly affected, particularly during explosive activities like sprinting, jumping, or changing direction. Non-sporting causes include lifting heavy objects, sudden movements, or fatigue. The severity of the strain determines the recovery timeline, which can range from a few days for a mild strain to several months for a significant tear.
Structured Rehabilitation for Full Recovery
The old advice of complete rest for muscle injuries is outdated. Current evidence supports early, controlled loading to promote optimal healing. I use a phased rehabilitation approach: the initial phase focuses on protecting the healing tissue while maintaining gentle movement; the intermediate phase progressively loads the muscle with strengthening exercises; and the final phase develops the power, speed, and sport-specific demands needed for a safe return to full activity. This structured approach not only speeds recovery but significantly reduces the risk of re-injury, which is particularly high in the first few weeks after returning to sport.
Common Symptoms
- Sudden sharp pain during activity
- Localised tenderness and swelling
- Bruising appearing within 24-48 hours
- Pain when stretching or contracting the muscle
- Weakness or inability to use the muscle fully
- A palpable gap or lump in severe injuries
How I Treat It
- Early controlled loading exercises
- Progressive muscle strengthening programme
- Soft tissue massage and release
- Running and agility drills for sport return
- Eccentric strengthening for injury prevention
- Sport-specific rehabilitation and testing
What to Expect
- Assessment to grade the severity of the strain
- Early management advice for optimal healing
- Progressive rehabilitation through three phases
- Regular strength testing to track recovery
- Sport-specific return-to-play testing
- Prevention programme to reduce re-injury risk
