Cracking Your Knuckles: Harmless Habit or Joint Damage?

Almost everyone has been told off for it. You crack your knuckles at the kitchen table and somebody warns you that you will end up with arthritis, or hands like claws by the time you are sixty. It is such a widely repeated warning that most people simply accept it. The cracking joints myth is one of the few pieces of health folklore that has actually been investigated properly, though, and the findings are reassuring — with a couple of genuine caveats worth knowing.
The belief
Repeatedly cracking your knuckles wears the joint out, damages the cartilage and causes arthritis in later life. Variants include claims about permanently enlarged knuckles, weakened grip and "loosening" the joints.
What is actually making the noise
For a long time nobody was certain. The leading explanation, now supported by real-time imaging of joints as they crack, is a process called tribonucleation.
Your finger joints are wrapped in a capsule containing synovial fluid, a slippery lubricant with dissolved gases in it. When you pull or bend the joint, you increase the space inside the capsule and drop the pressure. At a certain point a gas cavity forms suddenly within the fluid, and that rapid formation is what produces the pop.
Two everyday observations fit this explanation neatly. First, you cannot immediately re-crack the same knuckle — you have to wait around twenty minutes while the gas redissolves. Second, cracking does not hurt. Grinding cartilage would.
What the evidence actually shows
Researchers have compared habitual knuckle crackers with non-crackers on hand X-rays and clinical examination, and the consistent finding is that habitual cracking is not associated with developing osteoarthritis of the hands. There is also the delightfully stubborn case of a doctor who cracked the knuckles of one hand only, for decades, specifically to test the claim, and found no meaningful difference between his two hands at the end of it. It is a sample of one, but combined with the population studies it points the same way.
The picture is not perfectly clean. Some older work suggested habitual crackers might show slightly more hand swelling or lower grip strength, but this has not been reliably reproduced, and the effect — if real at all — is nowhere near the arthritis your nan was warning you about.
The practical takeaway: knuckle cracking is a habit, not an injury. If it bothers the people around you, that is a social problem rather than a medical one.
Why joints crack when you are not trying
This is the more interesting question, because most of the clicking, popping and grinding people ask me about is not deliberate at all. Knees that crunch on the stairs. A shoulder that clunks on the way up. Ankles that crack on the first step of the morning. Hips that pop when you roll over in bed.
Several different things produce these sounds:
- Cavitation, exactly as above — common in the spine and hips.
- Tendons or ligaments flicking over a bony prominence, which is why some hips and shoulders click rhythmically with a particular movement.
- Crepitus, the fine grating or crunching you often feel more than hear, from slightly roughened joint surfaces. It becomes more common with age and is frequently found in people with no pain at all.
- Stiffness after a period of stillness, which is why the noisiest part of the day is usually the first ten minutes of it.
None of these are inherently a problem. A noisy joint is not a failing joint.
When a click does deserve attention
Sound alone is not the signal. What matters is what comes with it. Get a joint assessed if:
- The click or pop is consistently painful, not just noisy
- The joint locks, catches or gives way — a knee that suddenly jams or buckles can indicate a meniscal or ligament problem
- The noise arrived suddenly after an injury, particularly with swelling or bruising
- There is swelling, heat or redness around the joint
- Movement is becoming progressively more restricted
Seek urgent medical advice for a hot, swollen, exquisitely painful joint with feeling generally unwell or feverish, which needs ruling out as a joint infection, and for any joint that is visibly deformed after trauma.
A separate word on necks. Repeatedly wrenching your own neck to make it crack is a habit I would gently discourage — not because of arthritis, but because it usually indicates you are chasing relief from stiffness that has a treatable cause, and because forceful self-manipulation of the neck is a poor idea in general. Address the stiffness properly and the urge tends to fade.
What to do instead
If you crack a joint because it feels tight, the tightness is the thing to work on. Regular movement through a full range, some strength work for the muscles around the joint, and breaking up long static positions all help far more than the momentary relief of a pop. The office workers I see around Merseyside often find that hand and wrist stretches, plus a few minutes away from the desk each hour, reduce the fidgety urge to crack considerably.
If a joint clicks and hurts, that is worth a proper look.
